June is Western NSW Integrated Care Awareness Month
Health Minister Jillian Skinner and Member for Bathurst Paul Toole meet the Integrated Care Team in Blayney
Chronic and complex disease affects nearly 40% of Australians and include illnesses such as diabetes, heart disease or lung disease. The integrated care team includes a range of health professionals and community services (GPs, community nurses, dieticians, physiotherapists, counsellors and pharmacists) that work together to create unique care plans for patients suffering from these conditions in the community. Western NSW Local Health District is leading the integrated care initiative in our region in partnership with the Western NSW Primary Health Network and Bila Muuji Aboriginal Health. Integrated care sites in Western NSW include Cowra, Molong, Cobar, Wellington, Dubbo, Blayney, Mudgee, Coonamble and Walgett and Lightning Ridge. In June we encourage people to identify early warning signs of chronic disease and seek help. Join the Conversation #myhealthmatters #yourhealthmatters
