This education session will explore evidence-based acute stroke and TIA management and pathways of care.

TIA has a triple end-point similar to AMI survivors and an elevated risk of death out to at least nine years.

It should be managed as urgently and comprehensively as an acute coronary syndrome.

Ischaemic stroke mortality is 17 per cent at 30 days in NSW and continues to improve ahead of other commonly fatal and disabling conditions.

The improvement is the result of implementing evidence-based practice at the bed-side and across the health system, beginning with pre-hospital screening and field diversion by ambulance.

Stroke Units confer 30 per cent improvement in mortality and discharge destination, and intravenous Alteplase thrombolysis < 4.5 hours reduces disability in selected patient’s by 30 per cent.

More recently endovascular clot retrieval < 24 hours has been shown to reduce disability by an additional 30 per cent in large vessel occlusions that have typically had a poor prognosis.

The acute stroke window is now 24 hours although eligibility for the hyper acute reperfusion therapies declines with passing minutes.

There are systems in place in Western NSW LHD to provide effective access to Stroke Units, thrombolysis and clot retrieval.

These rely on bypass or no stop/no stay pathways to minimise time lost in a time critical process.
