This is a GP, Practice Nurse and Practice Manager focussed event, however, all interested Nurses, Pharmacists, Medical Students, Specialists and Allied Health Professionals are also welcome to attend.

In order to get the most out of the workshop, we encourage all participants to commence the Advance Project online learning modules prior to the workshop. To access the online learning, please register for The Advance Project at www.theadvanceproject.com.au

  • Identify patients who will benefit from an introduction to Advance Care Planning (ACP) and/or palliative/supportive care needs assessment
  • Assess patients’ needs for ACP and palliative/supportive care
  • Evaluate patients’ requirements for follow-up and additional support to meet ACP and/or palliative/supportive care needs
  • Implement Advance Project resources in your practice

6.20pm -  Welcome & Introduction to electronic Palliative Approach Framework

6.30pm -  The Advance Project

8.30pm -  When to Refer - Local Service Profile

9.00pm -  Evaluation and Close
