This information session will provide an update on the changes to the Primary Health Reporting Dashboard for Practice Managers and Practice Staff. It will include a practical demonstration of the Primary Health Information Platform and explains how the platform relates to the PIP QI and clinical management of patients, with a population perspective.
This session will cover the following learning outcomes:
• Outline the basic features of the Primary Health Information Platform
• Identify the PIP QI 10 measures
• Relate how population health data can support clinical management decisions
