Changes to where you can access Specialist Referral Information from 1 July 2024
WNSW PHN is investing in better ways to support General Practice so that our communities receive the right care, in the right place, at the right time.
For Practices in Western NSW:
HealthPathways is a web-based health information site which helps clinicians guide patients through the complex local health system.
From 1 July 2024, WNSW PHN will be migrating up-to-date referral information to the Western NSW HealthPathways site so that you have access to all the resources and information you need in one place.
The webpage you are viewing now will become inactive as of 1 October 2024, when all referral information will be accessible through Western NSW HealthPathways.

For Practices in Far West NSW:
A new statewide digital referral management solution is being introduced to improve the referral experience of patients, referrers and clinicians in outpatient services.
Far West Local Health District (FWLHD) has partnered with eHealth NSW and HealthLink to enable electronic referrals (eReferrals) to be sent to select FWLHD outpatient services. An eReferral is an electronic document that can be sent electronically from one health care service to another, designed to be more sustainable than the current process of faxing, emailing or posting referrals.
HealthLink eReferrals will allow primary care general practitioners, specialists, nurses and allied health professionals to efficiently send secure eReferrals to clinics across the Far West Local Health District. They are designed to enhance the triage and health management of a patient’s journey, improve patient safety, and reduce specialist outpatient wait times.
