Registrations of Interest are being requested from GPs in General Practices currently working in the Western NSW Primary Health Network region of NSW, who visit resident patients in Aged Care Facilities, to deliver Telehealth for Residential Aged Care Facilities (TRAC) Program services.
The aim of the TRAC Program is to improve access to health professionals for residents of aged care facilities with chronic and complex health conditions by funding a model of care where a General Practitioner (GP) provides consultations via video conferencing to aged care residents in Residential Aged Care Facilities (RACFs) in the communities within the Western NSW region.
The Western NSW Primary Health Network (WNSW PHN) is requesting Registrations of Interest from General Practitioners currently working in General Practices that provide face to face consultations to residents of aged care facilities to also, be able to provide Telehealth video consultations to these residents.
This contract will be a one-year term, 1 July 2020 to 30 June 2021 with the opportunity of an extension for a further six months. If you have any questions regarding the Registration of Interest, please contact email@example.com
