Kidney Health Australia’s primary care education program in partnership with Western NSW Primary Health Network is pleased to present an interactive case study based Active Learning Module (ALM).

There are three parts to an ALM all for CPD point eligibility:

  1. Pre-disposing activity and pre-reading
  2. Six hours of education (both F2F sessions)
  3. Reinforcing activity (see over for more details)

Note: Participants can also self-report for Category 2 CPD points if not completing all components of the ALM

Presenters:

  • Session 1 - Jeniffer Fiore-Chapman and Colin McClintock (Nephrologists)
  • Session 2 - Colin McClintock, James Collett and Jeniffer Fiore-Chapman (Nephrologists)

Program:

Session 1:

  • CKD Issues for GP’s – 2 case studies
  • Managing Stage 4 and 5 CKD: working with the Nephrologist

Session 2:

  • CKD Issues for GP’s – 2 case studies
  • Decision making and symptom control in End Stage Kidney Disease 

Learning outcomes:

  • Describe the role of the GP in preventing and detecting CKD in the primary care setting and individual cases
  • Determine when to refer patients with CKD to a nephrologist according to the recommended clinical indicators
  • Demonstrate the ability to correctly stage CKD and manage CKD complications common to each stage
  • Recognise the need for dosage reduction or cessation of commonly prescribed drugs in stage 4 and 5 CKD
  • Implement a practice-based system, to improve patient safety by identifying patients at higher risk of both CKD and established CKD
