What is the ITC program?

The Integrated Team Care (ITC) program supports Aboriginal & Torres Strait Islander people with chronic (long term illness) conditions like diabetes and asthma, to name a few.

Priority is given to those who have complicated care needs and require support to manage their health conditions.

The ITC Program:

  • Provides access to Care Coordination support
  • Provides access to Aboriginal Outreach Worker support
  • Assists you to understand your health condition/s and medications
  • Provides access to some medical equipment
  • Links to other helpful services and programs

* Please note there are Federal Government guidelines which we must adhere to when determining eligibility and assessing access to the ITC program.




Who is eligible to access the program?

Aboriginal & Torres Strait Islander people who:

  • have one of the following chronic diseases:
    • Diabetes and eye related conditions
    • Cardiovascular
    • Chronic Kidney Disease
    • Chronic Respiratory Disease
    • Cancer
    • Liver Disease
  • have a current GP Management Plan (GPMP) and Team Care Arrangements (TCA)
  • have a GP referral
  • live in the Western and Far West NSW region



I’m a patient with a chronic health condition. How do I access care through the ITC program?

The first step is talking to your usual Doctor (whether that's at an AMS or a mainstream practice) about the ITC program. They will assess your eligibility and refer you to the program if you meet the eligibility requirements.




Who are the Providers of the ITC program?

Eligible Aboriginal residents in Western and Far West NSW who attend their local AMS to access care for their chronic health condition are provided with care through the ITC Program on-site. These providers are:

  • Orange Aboriginal Medical Service
  • Yoorana Gunya Family Healing Centre Aboriginal Corporation
  • Condobolin Aboriginal Health Service
  • Peak Hill Aboriginal Medical Service
  • Dubbo Regional Aboriginal Health Service
  • Coonamble Aboriginal Health Service
  • Walgett Aboriginal Medical Service
  • Brewarrina Aboriginal Health Service
  • Bourke Aboriginal Health Service
  • Maari Ma Primary Care Health Service (Broken Hill and Balranald)
  • Coomealla Health Aboriginal Corporation

Meanwhile, Western NSW Indigenous residents attending their local GP to access care for their chronic health condition will be referred, if eligible, to their regional provider, as per the following region breakdown based on Local Government Areas:
